do not suffer from any of the listed conditions.
·
If you are aged 16 or 17 then you will need your medical form signed
by a parent or guardian and a letter of consent to be presented when
you are booking in on the day of your parachute jump.
· If you are aged 18 to 39 then you can sign your own medical
form.
· If you are aged 40 or over then the enclosed medical form must
be signed by a doctor after they have read the notes on the back of
the form.
There
are various weight restrictions at various skydive centres. The general
rule of thumb is that weights up to 14 stone present no problem. If
you are over 14 stone then just email us at tandemskydive@yahoo.com
with the type of jump you would like to do and which centre you would
like to make your jump and we will check with the centre.
